Role Description
This is a contracted role at Peace of Mind Mental Health, Inc. located in Cutler Bay, FL. The Targeted Case Manager will be responsible for performing case management services, which include assessing clients' needs, developing care plans, coordinating services, and advocating for clients' rights and access to resources. The Targeted Case Manager will also maintain accurate and timely documentation, collaborate with healthcare professionals and social service agencies, and provide support and guidance to clients and their families.
Qualifications Bachelor's degree in Social Work, Psychology, Counseling, or a related field
Experience in case management, social services, or a related fieldKnowledge of mental health disorders, treatment options, and community resourcesStrong organizational and time management
skillsExcellent communication and interpersonal
skillsAbility to work independently and as part of a teamCultural sensitivity and awarenessProficiency in relevant software applicationsValid driver's license and reliable transportationBilingual
skills (English/Spanish) are preferred
