Lpn-hospice Homecare- Saratoga/washington/montgomery Counties
The Community Hospice has a wonderful opportunity for a per diem LPN to work (Hours as needed between Monday-Friday) as a member of our Hospice Home Care Team caring for patients and families residing in either parts of Saratoga/Washington/Montgomery county.

What You Will Do
Our LPNs coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families.The LPN is responsible for caring for patients in in their homes.Responsible for providing personal care as well as tasks within LPN scope of practice as assigned.This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
What You Will Need
Valid NYS LPN License1-2 year's LPN
experience preferably in acute care, caring for seniors, or in Homecare or HospiceValid driver license and reliable vehiclePrevious home care / hospice
experiencePrevious
experience caring for geriatric patientsStrong interpersonal
skills to establish and maintain relationships with patients, families, physicians and coworkers

Pay Range:$24.00 - $30.10
Pay Is Based On
experience,
skills, and
education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base
salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on
location.
