Role Overview & Specifications
We are looking for three community focused nurse case managers within our urgent response team, who will be supporting people as they leave ED and the Assessment units.
You will assess and deliver treatments to people in ED and their own homes who are experiencing an exacerbation in their health. You will work as part of a bigger team including consultants, GPs, and ACPs as we deliver hospital at home interventions in the patients home. You will therefore, have a passion for community nursing and urgent care pathways .
You will join the Integrated Care Service (iCares) which works across Sandwell Community providing rehabilitation, case management and admission avoidance.Due the recent expansion of the service and Directorate, there is the opportunity to bring your leadership and clinical skills to help guide our evolving service.
Within the Team we have ACPs, Occupational Therapists, Physiotherapists, Community Matrons, Speech and Language Therapists, District Nurses, Assistant Practitioners and Rehab Support Workers, delivering integrated care and supporting recovery.
