Job Description

Under supervision, this peer counselor provides peer empowerment services to the patients of the Mobile Crisis Team. Works in close collaboration with staff, patients and other caregivers in planning and implementing treatment and wellness plans. Educates and empowers peers to advocate for themselves, resolve issues or problems, and plan for their futures by serving as a role model.

Provides supportive peer counseling with respect to mental health, substance abuse, rehabilitation strategies and self-help opportunities. Making home and community visits is an integral part of the peer role. Ability to travel independently is required. Assists the patients in identifying their own individual warning signs of relapse and identifying and developing individual coping strategies. Participates in interdisciplinary treatment team meetings and assists with determining and implementing a comprehensive plan of care. Leads or co-lead groups including, but not limited to dual diagnosis issues, relapse and recovery, and socialization, if necessary. Clarifies issues for the patient and assists with the process of referral for treatment, rehabilitation, housing and supportive services. Follow-ups to determine whether services were provided and used. Escorts to appointment when necessary.

Minimum

Qualifications

For Assignments In Mental Health

Satisfactory and documented completion of a formalized Peer Counselor/Specialist Training Program; and Certification as a Peer Specialist (NYCPS) issued by the Peer Specialist Certification Board.

For Assignments In Substance Use

Certification as a Recovery Peer Advocate (CRPA) issued by the New York Certification Board.

For All Other Assignments

A High School Diploma Or Its

educational equivalent; and Satisfactory and documented completion of a formalized Peer Counselor/Specialist Training Program; or One (1) year of full-time satisfactory

experience working with consumers of mental health services as a peer

counselor/specialist, peer advocate or in related human services.

Assignment Level Ii

In Addition To Meeting The Qualification

requirements of Peer Counselor Level I, assignment to Level II requires:

One (1) additional year of

experience, as described in three (3) above.

Department Preferences

Experience in dealing with mentally ill patients, knowledge of community agencies.
