We are searching for an energetic Physical Therapist to join our team full time. Enjoy working one on one with your patients in the most functional environment possible --- their home. Qualifications:Graduate of an approved physical therapy curriculum.Possesses a current license to practice as a physical therapist in Ohio.

Responsibilities:Assists the physician in evaluating clients by applying approved diagnostic and functional ability tests.Treats clients to relieve pain and/ or develop or restore function to maintain maximum performance.Makes appropriate arrangements for outpatient client services for services which cannot be given in the home.Records and reports to the physician the client’s reaction to treatment and/ or changes in the client’s condition.Instructs clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, crutches, wheelchairs, etc).Instructs the family in the client’s total physical therapy program.Attends care conferences and Agency meetings as necessary.Supervises the physical therapy assistant.Prepares an assessment of the client’s condition and periodic evaluations and summaries, develops a care plan and writes clinical/ progress notes and submits them to the Agency weekly.Communicates effectively with all those providing care.Participates with staff, client and physician in discharge planning activities and completes the physical therapy discharge

summary.Organizational Relationship:Reports directly to the Director of Rehabilitation Services.Functional Abilities:Must be able to read 12 point type or larger.Must be able to communicate verbally and in writing in a manner understood by most persons.Must be able to stoop, bend and be able to lift and transfer clients.Must be able to travel to client’s residences.Special

requirements:Must have a reliable car, car insurance and a valid driver’s license.

Job Type: Full-time/PRN

Salary: $109,200 per year plus incentive opportunities.
