Responsibilities

Assist consumers in skill development,Collaborate and develop IAP’s (Individualized Action Plans) with consumers and other staff when appropriateAssist with community integrationProvide medication administration,Support activities in the community,Develop short term goals with consumers to assist with integration into the community.

Requirements

Bachelor's degree in either Psychology or Social Work or a related fieldValid driver's licenseReliable vehicleIdeally 2-3 years prior

experience working with adults having severe and persistent mental illness
